TOURNAMENT Liability Waiver

Our online Tournament Liability Waiver must be completed before the tournament by all players, coaches, managers (i.e. anyone needing access to the fields or dugouts for a team). Paper versions of the form will be accepted at tournament check-in only and paper copies will not be provided at the tournament. We strongly encourage all participants to complete the waiver electronically below. Any participant that does not complete the waiver electronically will need to print their own paper copy of the waiver and turn it in at tournament check-in. Any player, coach, manager, etc. that fails to submit a Liability Waiver will be prohibited from participating or accessing the fields, dugouts, and other team areas.
